Senior Living AI Screening
Twelve questions. Six AI governance and impact areas.
These questions ask whether AI in your community supports dignity, what matters to residents, and staff time for care.
A quick look at AI inventory, shadow AI, staff rules, vendor purchases, human review, data safeguards, and monitoring of results. This self-reported screen is not the independently observed AISureCare facility baseline.
Do not enter resident names or any resident information. The questions ask how your community operates, not about any person.
Preview only. Nothing you enter here is saved or sent.
Your readiness results
These are self-reported screening results, not an observed AI footprint baseline, an audit, or proof that deployed AI is effective.
Overall readiness score
Score by area
Each area has two questions and is scored from 0 to 6: Strong (5 to 6), Developing (4), or Needs Attention (0 to 3). An area with a Not sure answer always shows Needs Attention.
Where to start
These are your lowest self-reported AI oversight areas. Consider an independent facility baseline to validate actual conditions.

Services that fit these results
Each service can start on its own. The ones marked below relate to your lowest areas.
The score reflects your answers about AI governance and outcome monitoring. It is not independent verification, compliance certification, or a clinical assessment.
Score ranges are an early version and may change as the assessment is refined.
Talk through your results
Discuss the scores, what AI may already be in use, and whether an independent facility baseline or AI performance follow-up makes sense.
Questions
About the assessment
What if I am not sure of an answer?+
Choose Not sure. It counts as zero points, so that area shows Needs Attention. Areas with a Not sure answer appear first in Where to start, because finding the answer is often the most useful next step.
Can I go back and change an answer?+
Yes. Use Back to return to an earlier area, or select Change on the review screen. Your answers stay in place while you move between steps, until you leave or refresh the page.
Will my results be emailed to me?+
Not at this time. Your results appear on screen when you finish. Use Print or Save These Results to keep a copy as paper or a PDF.
Is the assessment free?+
Yes. The assessment is free. If you want to talk about services afterward, you can book a call.
