Grant-Funded AI Baseline Screening
Twelve questions. Six baseline readiness areas.
Your grant says what the project will achieve. These questions ask whether you can capture where things stand today, before the AI goes live, so you can show what changed for the people you serve.
It looks at your measures, current workflows, the starting point for people and staff, existing AI and data, baseline timing, and how results will be compared. It is not an audit or grant eligibility test.
Do not enter patient or resident names, or any information about a person. The questions ask how your organization operates.
Preview only. Nothing you enter here is saved or sent.
Your readiness results
These self-reported answers indicate possible readiness gaps for independent AI governance and grant-outcome reviews; they are not a project evaluation.
Overall readiness score
Score by area
Each area has two questions and is scored from 0 to 6: Strong (5 to 6), Developing (4), or Needs Attention (0 to 3). An area with a Not sure answer always shows Needs Attention.
Where to start
These areas scored lowest in the free screen. They are discussion prompts, not findings from an observed project baseline.

Services that fit these results
Each service can start on its own. The ones marked below relate to your lowest areas.
This self-reported AI-project readiness score is not an official RHTP, grant, regulatory, or clinical compliance determination.
Score ranges are an early version and may change as the assessment is refined.

Questions
About the assessment
What if I am not sure of an answer?+
Choose Not sure. It counts as zero points, so that area shows Needs Attention. Areas with a Not sure answer appear first in Where to start, because finding the answer is often the most useful next step.
Can I go back and change an answer?+
Yes. Use Back to return to an earlier area, or select Change on the review screen. Your answers stay in place while you move between steps, until you leave or refresh the page.
Will my results be emailed to me?+
Not at this time. Your results appear on screen when you finish. Use Print or Save These Results to keep a copy as paper or a PDF.
Is the assessment free?+
Yes. The assessment is free. If you want to talk about services afterward, you can book a call.
